Figures 8.8 and 8.9 show two classic examples, both present in modern mines,
which should be evaluated as discussed below.
8.3.1 Pipe-Dam Inter-Dependency
Figure 8.8 shows a common example of a tailings line on top of a dam. It may
be exposed to hazards (see Chap. 9 for Hazard Identification) such as traffic (with
construction equipment), joint failures, etc. Its probability of failure may be seen as
a series system, including chances it will not be detected for a certain number of
hours. At that point the domino effect can develop. If the spill can migrate toward
the downstream face of the dam, erosion, then invasion of berms, then sloughing will
occur. Finally, as the phenomena progress, a dam break which could probably have
been reported in old statistics as due to stability will occur.
